LifeClinic (“we” or “us”) respects privacy and is committed to protecting personal information. This Privacy Policy (the “Policy”) describes the information we may collect about you through our websites, mobile applications, and all other services we provide or when you otherwise interact with us (collectively, the “Services”), how we use and disclose that information, and the privacy choices we offer. Use of our Services is also subject to our Terms of Use. By providing us with personal information or using the Services, you expressly consent to the information-handling practices described in this Policy. If you do not agree with this Policy, please do not use the Services or provide us with your information.

While we always aim to provide complete and transparent information about how we process your personal information, we reserve the right to amend or update this Policy from time to time, or to create additional policies, in order to accurately reflect changed circumstances or new legal requirements. As a result, it is important that you read this Policy closely so that you are fully aware of how and why we are using your personal information. If we make changes, we will notify you by posting the updated policy on our website (and other relevant areas within our Services) and revising the “Last Updated” date above. Your Health Information

Information that reveals your medical or health condition or status, or that otherwise relates to your receipt of medical or health care (collectively, “health information”) that we may receive about you is collected, used, disclosed, and protected as described in LifeClinic’s Notice of Privacy Practices, which is available at our facilities and on our website. To the extent applicable, this Policy supplements our Notice of Privacy Practices. In the event of a conflict between the terms of this Policy and the terms of the Notice of Privacy Practices with respect to your health information, the terms of the Notice of Privacy Practices shall control.

Disclosure Regarding Use of AI

Our website may use chatbots and other artificial intelligence technologies to assist visitors. These technologies are automated systems and are not real people. Responses generated by chatbots or AI tools are computer-generated and may not always be accurate or complete. If you would like to interact with a real person, please contact us using the email address or phone number provided at the end of this Policy.

Collection of Personal Information

For the purposes of this Policy, “personal information” is any information that identifies, relates to, or can be used to contact a particular individual.  

A. Information You Provide. The personal  information we collect from you can be categorized as follows:

  • Contact information – first name, last name, email address, mailing address, telephone number.
  • Communication information – copies of communications and inquiries you have submitted to us, including through email, calls, chatbots, and features available on our Services.
  • Marketing information – details regarding informational and promotional materials you may have requested or received from us, your receipt of promotional communications, and information on your marketing or communication preferences. Device Information - details of the computer or devices (including mobile devices) you use to access the Services. For example, we may collect and analyze information such as (a) IP addresses, location information (e.g., zip code), unique device identifiers and other information about your mobile phone or other mobile device(s), browser types, browser language, operating system, the state or country from which you accessed the Services; and (b) information related to the ways in which you interact with the Services, such as: referring and exit pages and URLs, platform type, the number of clicks, domain names, landing pages, pages and content viewed and the order of those pages, the amount of time spent on particular pages, the date and time you used the Services, the frequency of your use of the Services, error logs, and other similar information. As described further below, we may use third-party analytics providers and technologies, including cookies and similar tools, to assist in collecting this information.
  • Location Information - information about your location, including general information (e.g., IP address, zip code) may use that information to customize the Services with location-based information, advertising, and features. For example, if your IP address indicates an origin in Chanhassen, MN, the Services may be customized with Chanhassen-specific information and advertisements. In order to do this, your location information may be shared with our agents, vendors or advertisers. If you access the Services through a mobile device and you do not want your device to provide us with location-tracking information, you can disable the GPS or other location-tracking functions on your device, provided your device allows you to do this. We collect this information when you visit or navigate our website, fill in forms through use of our Services, communicate with us (including by phone, email, or otherwise), visit or engage with our social media pages, participate in surveys or sponsored activities, or otherwise provide us with personal information.

Please note that we may aggregate or anonymize the foregoing types of information such that they are no longer capable of identifying you, in which case they are no longer considered “personal information.”  We may also collect information about you from third-party sources and information about you that is publicly available.

B. Automatically Collected Usage and Device Information. Like most website operators, we use various technologies to automatically collect information about visitors.  Those technologies include:

  • Pixels, tags, and web beacons - Our website may also use “pixels,” “tags,” or “web beacons.” When we refer to “pixels,” we are generally referring to all of these types of technologies. These technologies are small pieces of code that run when a page or email is loaded. They are used to monitor the behavior of the visitor or email recipient—such as what icons were clicked or whether links in an email were opened—and gather analytics.  For example, when you open our homepage, a pixel may run and generate information based on the visit, and then this information is processed by us and our vendors. Pixels work in conjunction with cookies to let us know what portions of our website are of interest to you and to help us provide you with tailored information from our website. C. Information from Third Parties. We may obtain additional information about you from third parties such as marketers, partners, researchers, social media platforms, and others. We may combine information that we collect from you with information about you that we obtain from such third parties.

Our Marketing Efforts

We may send you direct marketing communications from time to time including news, updates, offers, and other promotions relating to our services and activities. We will only send marketing messages where the law allows us to do so and, in the event that you no longer wish to receive marketing from us, then you can always unsubscribe using the option provided in the footer of the communication you have received or by contacting us using the information below.

SMS/Text Messaging

By providing your mobile phone number and opting in to receive text messages from LifeClinic, you consent to receive recurring automated marketing and informational text messages to the mobile number provided. These messages may include information about our services, upcoming events, scheduling, updates, and promotional offers.

No mobile information will be shared or sold with third parties or affiliates for marketing or promotional purposes.

Message frequency varies based on your interactions with us and may range from occasional messages to several messages per month depending on your preferences and inquiries. Message and data rates may apply. Please check with your mobile carrier for details regarding your specific plan. Carriers are not liable for delayed or undelivered messages.

To stop receiving text messages from us at any time, reply STOP to any message you receive. You will receive a one-time confirmation message confirming your opt-out. After opting out, you will no longer receive promotional text messages from LifeClinic unless you opt in again. Disclosure of Personal Information

When the information we collect about you is aggregated, anonymized, or otherwise does not identify you, we may use that information for any purpose or share it with third parties, to the extent permitted by applicable law.

In addition, we may disclose your personal information to the following types of entities:

  • Business partners, service providers, and third-party vendors necessary for the performance of any agreement we enter into with you. For example, our IT providers, payment processing providers, shipping providers, customer contact center services, insurance providers, and document storage providers.
  • Service providers we use to facilitate our business operations and administration. These third parties have access to your personal information only to perform specific tasks on our behalf and are obliged not to disclose or use it for any other purpose. For example, our service providers include (i) IT and system administration providers, (ii) data storage providers, (iii) vendors to facilitate payments and payment processing, (iv) vendors that verify relevant details about you. Please note that our parent company and affiliated entities may act as service providers on our behalf.
  • Marketing and advertising vendors that may assist with lead generation, hosting information relating to potential customers, marketing automation, advertisement placement and targeting, and marketing campaigns and communications.
  • Analytics vendors in order to understand our website traffic and usage patterns, optimize our Services, and identify potential new customers. We use appropriate administrative, technical, and physical measures to protect your personal information from loss, theft, and unauthorized use, disclosure, or modification. Please be aware that no data transmission over the Internet is 100% secure. While we strive to protect your personal information, we cannot ensure or warranty the security of any information you transmit to us and you do so at your own risk.

Social Media Platforms

Certain sections or functionalities on our Services may permit you to share information on third party social media sites or platforms such as Facebook, Instagram, LinkedIn, Twitter, Google+, or other similar sites (collectively, “Social Media Sites”). LifeClinic does not own or control such Social Media Sites, and posting your information on Social Media Sites is subject to the third party’s Online Privacy Policy and other legal terms, which may not provide privacy protections with which you agree. LifeClinic is not responsible for any act or omission of any Social Media Site, nor are we responsible for the consequences of your choosing to share your information on Social Media Sites.

Your Choices About Personal Information

How You Can Access and Update Your Information. You may update or correct information about yourself at any time or by emailing us at supportteam@lifeclinics.life.

Marketing Emails. As required by applicable laws, you can opt-out of receiving promotional emails from us by clicking the “opt out” link in any such promotional emails and following the instructions provided.

Text Messages. As required by applicable laws, you can opt-out of receiving text messages from us by replying “STOP” to such text messages and following any other instructions provided. Note, we may send a follow-on message to confirm your opt-out request. We may also continue to send you informational communications via text message, where the law allows us to do so.

Tracking Technologies. Depending on your browser or device, you may have the option to set the browser to accept all cookies, reject all cookies, notify you when a cookie is set, or delete cookies.  Each browser and device are different, so we recommend you evaluate the tools and settings available in your browser or device, as well as any available instructions for the same.  Please note that if you disable or delete cookies, you may not be able to access or use certain features of our website. Google Analytics. As discussed above, we use Google Analytics in connection with our Services. Do Not Track Mechanisms. Please note that our Services does not honor “Do Not Track” signals, and such signals will not impact the operation of our website.

Rights Under the Laws of Your State

The laws of the state in which you reside may afford you certain rights with respect to your personal information. The above sections of this Policy describe the categories of personal information processed by us, the purposes for the processing of such personal information, the categories of personal information that may be disclosed to third parties, and the categories of third parties to whom we may disclose personal information.

Depending on the laws of your state, you may have the right to request (1) that we confirm that we are processing your personal information, (2) access to the personal information we process, (3) correct inaccuracies in the personal information we process, taking into account the nature of the personal information and the purposes of the processing of such personal information, (4) delete the personal information provided by you or obtained about you, subject to certain exceptions under applicable laws, (5) a copy of the personal information that we process, subject to certain limitations and exceptions, (6) opt out of the processing of your personal information for the purposes of targeted advertising, the sale of personal information, or profiling in furtherance of a decision that produces a legal or similarly significant effect, or (7) direct us not to make any sale of certain personal information to third parties.

LifeClinic does not engage in any “sales” of personal information as defined by applicable state privacy laws. We also do not collect or process any “sensitive” personal information in a manner that would be regulated by any state privacy laws. We further do not use your personal information to profile you or automatically make decisions in a manner that has a legal or significant consequence.

In order to exercise any of the above-described rights, you or an agent acting on your behalf must send an email to supportteam@lifeclinics.life with the subject “Privacy Request” or call us at 1-952-658-2826. We may require you or your agent to provide us information that enables us to authenticate your request. If we are not able to authenticate your request using commercially reasonable efforts, we may decline to comply with your request. If we refuse to take action on your request, we will notify you of the same. If you wish to appeal our decision, please send us an email at supportteam@lifeclinics.life with the subject “Privacy Appeal.”

Additional Information for California Residents

California Civil Code § 1798.83 (California’s Shine the Light Act) further permits California residents to request certain information regarding our disclosure of personal information to third parties for their direct marketing purposes. If you are a California resident, you may ask us to refrain from providing certain of your personal information to certain of our affiliates and other third parties for their marketing purposes. Additional Information for Nevada Residents

Under Nevada law, Nevada “consumers” may request that certain “operators” of internet sites and online services refrain from the “sale” of their “covered information” to third parties (as such terms are defined under Nevada law).  Our disclosures of your information to third parties (as described above) do not qualify as a sale under Nevada law. Nonetheless, you may contact us at the information below to make a request under the laws of the state of Nevada.

Children’s Privacy

Our Services are intended for a general audience and are not directed at, or intended for use by, children under the age of 13 years. Accordingly, we do not knowingly collect personal information from children under age 13. Should we discover that a child under the appropriate age provided his or her personal information, we will use that information only to respond to that child and inform him or her that we must have parental consent before receiving such information. LifeClinic is headquartered in the United States and, as a result, the information you provide to us may reside on servers or workstations located in the United States. You understand and consent that information you provide may be transferred to the United States. When we transfer personal information to the United States, we will implement appropriate and suitable safeguards to protect such information as may be required by applicable data protection laws.
